Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.

From admission to discharge and beyond, McAlester Regional Health Center’s Case Management Department provides a wide range of assistance and resources to patients and their families. Case Managers are involved in every aspect of your hospitalization, and help ensure a collaborative effort to provide quality care throughout your stay.

The Case Management process emphasizes communication and teamwork between patients, nurses, social workers, physicians, other practitioners, and caregivers in the community. We’re here to provide a continuum of care through effective resource coordination, and hope to achieve optimal health, ensure access to care, and make sure resources are being appropriately utilized.

Sometimes, healthcare needs fall outside of the hospital. Our Case Management Department ensures patients are still receiving quality healthcare where they need it through McAlester Regional Health Center’s Transitional Care Clinic. We can assist with in-home care, or even provide for educational needs for up to 30 days after discharge. For more information on care after discharge, please visit our Transitional Care page.
