Medical Records

McAlester Regional Health Center is here to meet all of your healthcare needs, including copies of medical records. You can request these by mail or in-person from our Health Information Services (HIS) department. Simply bring or mail in a valid photo ID (see list below) and provide an “Authorization for the Release of Health Information” form signed by the patient or legal guardian.

Patients needing to obtain a copy of their x-rays will need contact the Imaging Department with your ID and a completed authorization form. If you’re requesting a CD, the wait time should only be about 15 minutes, whereas films can take up to an hour and a half.

As of 12/1/2014, McAlester Regional Health Center will charge 50 cents for each page if the request is for more than five pages. Requests five pages or fewer are free of charge. Records meant for your physicians are also free, and they can contact the HIS department directly to make a request. Physicians should fax requests to (918) 421-8163. One received, we will fax the records to them. Charges for radiology films will be at the discretion of the Health Information Services department. Please allow up to 30 days from the date of discharge for record processing.

  • State issued ID
  • Current school ID
  • Military ID
  • Veteran Administration ID
  • Valid current passport

  • Legal guardianship papers
  • Divorce decree
  • Death certificate

For an even easier way to access lab results and radiology reports that were completed in the past 200 days, consider using McAlester Regional Medical Center’s Patient Portal. To sign up for this service, you will need your medical record number. This is the “M” number located on your patient bracelet or patient sticker. Make sure to enter the “M” and all of the zeros when registering, and afterwards enjoy access to your medical records from wherever you need them!
