Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.
From admission to discharge and beyond, McAlester Regional Health Center’s Case Management Department provides a wide range of assistance and resources to patients and their families. Case Managers are involved in every aspect of your hospitalization, and help ensure a collaborative effort to provide quality care throughout your stay.
The Case Management process emphasizes communication and teamwork between patients, nurses, social workers, physicians, other practitioners, and caregivers in the community. We’re here to provide a continuum of care through effective resource coordination, and hope to achieve optimal health, ensure access to care, and make sure resources are being appropriately utilized.
Our Case Management Department helps patients prepare for a safe transition from the hospital to the next stage of their care. Case managers work with patients, families and the healthcare team to coordinate discharge needs and connect patients with appropriate resources and services.
Assistance may include coordinating home health services, medical equipment, rehabilitation or skilled nursing care, follow-up services and other resources needed after discharge.
